REQUEST - Practice Facility Inspection Report

FEE: Contact the Board's Practice Facility Coordinator

The Board will provide inspection information on any facility upon request. The Board's response will contain information on the most current inspection. This allows the consumer to make an informed decision regarding their choice of a veterinary professional.

Inspections occur every two (2) years. A new practice is inspected prior to opening and again the following year, then every two (2) years thereafter.

REQUEST - Data Request

FEE: Based on quantity of information ($0.06 per listing or a minimum of a $52 charge)

1. Request may be received via mail, fax or email
2. Include all desired elements; the listing may include the veterinarian or veterinary technician name, facility name, facility address, phone/fax number and county
3. Include your contact information (company, phone/fax number, mailing address and e-mail address)

NOTE: If you are requesting data towards the end of a calendar year, you may want to consider delaying your request until the first of March when our renewal window has closed and information is most current.

Once your data request has been received, please allow one (1) week to receive your estimate of charges. Once your payment has been received by the Board office, please allow another week to receive the data via e-mail or US mail.
